Referrals in our catchment area
We aim to make it as simple as possible for someone to make a self-referral. Self-referrals can be made by an individual, their family, carer or loved one.
We welcome self-referrals for adults with a life-limiting condition, living in our catchment area. We help oversee specialist palliative care and give advice on pain and symptom control. For example, if you are currently receiving palliative care under your local GP or district nurse, we may be able to support you.
If you are not sure, you may wish to have a conversation with your GP about whether you could be referred to our services.

You can refer yourself as a patient or a family member, carer or loved one can make a referral on behalf of someone else.

What happens after I have made my referral?
We will contact the referred person once initial assessment of the completed referral form takes place.

Is hospice care only for people with cancer?
No, hospice care (palliative care and end of life care) can benefit people who have been diagnosed with any illness that may shorten life.
Palliative care can also benefit their families and friends.
